  • Notifiable Condition 1.3.6.1.4.1.19376.1.3.1.1.1
  • Case Identification 1.3.6.1.4.1.19376.1.3.1.1.2 and/or
  • Outbreak Identification 1.3.6.1.4.1.19376.1.3.1.1.3
Notifications SHALL be present when dictated by local public health requirements.
